As I am a high profile target with the UK government, detained under the Mental Health Act for having dissenter views. What streaming services can I use for dissenters that want to maintain full privacy from Google?

You can consider using one of the invidious instances, or deploy one on your own that routes through proxies
I like opentune as a music client, but I use it logged in, which isn’t the best for privacy. In my situation, I’m fine with google tracking what music I hear as long as that’s the only thing the client shares with google.
If you are actually targeted, your phone should not contain any apps that are not truely required.
But as you mentioned mental health, anxiety often makes people misjudge their digital risks and taking more extreme measures that make their devices less usable to protect against nonexistent threats. You could talk about that with someone you trust who understands your situation if you struggle with finding a balance between privacy and usability.
I think I’ll carry on using Musify, which meets my basic needs. There’s no login for that app, so it’s useful. I’ll try the Invidious proxies though. Thank you.
